Foothills Community Health Care, Inc. Receives Patient Centered Medical Home (PCMH) Excellence Award

The Patient-Centered Medical Home (PCMH) is a care model that places patients at the heart of healthcare delivery. To achieve PCMH recognition, practices must meet criteria across six core concept areas. Research consistently shows that PCMHs enhance care quality, improve the patient experience, boost staff satisfaction, and help reduce overall healthcare costs.

Foothills Community Health Care has demonstrated its commitment to continuous quality improvement and a patient-centered approach by successfully meeting all 40 required criteria, along with at least 25 elective credits across the six concept areas.

As a result, Foothills Community Health Care has earned PCMH recognition for 2025 at all three of its locations—Clemson, Easley, and Anderson. Notably, the Easley location also received the PCMH Excellence Award, which provides additional funding for practices that achieve exceptional standards in patient-centered care.
